Vancouver Community of Care

Vancouver Community (VC) Health Services provides primary and community care to Vancouver’s 650,000 residents. VC delivers services across the life span through specialized programs for substance use, mental health, the frail and elderly and public health.

The Department of Family and Community Practice at VC has over 400 primary care physician members, working in acute care, community health and private practice. The department promotes collaboration among physicians to facilitate acute to community patient transitions. The Department, in partnership with the VCHRI, provides research support for physicians and staff across Vancouver Community.

VC research focuses on designing innovative clinical and health service interventions, program evaluation, needs assessments and measuring service utilization. VC selected Substance Use & Mental Health and Aboriginal Health as research priority themes for 2017-2020, acknowledging the urgent need for evidence to provide better care in these areas.
