By submitting this form, the team lead confirms that they have shared the project information with their manager, discussed the proposed practice change and its implications, and secured their manager’s support for the team’s participation. If the project is funded, the manager has agreed to work with team members to accommodate requests for scheduled time to contribute to the project.

Upon submission, the team lead further agrees to communicate any changes to the team or to participation, take part in all scheduled workshops and preparatory activities, complete the online KT Challenge evaluation surveys, and encourage team members to complete the surveys and learning activities.
